Key Responsibilities\n

• Deliver virtual educational support and counseling services to patients through telehealth platforms, ensuring high engagement and accessibility.

• Act as the primary point of contact for patients, coordinating communication between them and healthcare professionals, hence facilitating comprehensive care.

• Assess and understand each patient’s unique needs, preferences, and goals to provide a seamless and personalized experience that guides them through their pathway to treatment.

• Educate patients about program policies and procedures while meticulously documenting all interactions in clinical charts to maintain a detailed account of their treatment journey.

• Regularly update patient records and progress notes, ensuring that all documentation is accurate and provides insights into patient status and interventions.

You will work with a diverse client base, including adults facing various mental health challenges such as depression, anxiety, and trauma. Qualifications\n

• A Master’s degree in Social Work or Counseling from an accredited institution.

Compensation and Benefits\n
