Key Responsibilities\n

As a Licensed Professional Counselor within our organization, your day-to-day tasks will include:

• Conducting comprehensive patient intakes, including diagnostic assessments and functional evaluations, to inform each patient’s treatment plan.

• Providing brief, solution-focused interventions tailored to the unique needs of each patient, utilizing evidence-based practices like cognitive-behavioral therapy and motivational interviewing.

• Regularly reviewing and adjusting treatment plans in response to patient needs and treatment progress, while maintaining compliance with Federally Qualified Health Center (FQHC) guidelines.

• Collaborating with primary care providers to identify mental health patients who may benefit from integrated behavioral health services, ensuring appropriate referrals when specialty care is needed.

• Initiating follow-up communication to assess patient well-being and treatment efficacy, making necessary adjustments to therapeutic approaches.

• Documenting patient progress and treatment details accurately in clinical records, essential for ongoing monitoring and compliance with healthcare regulations.

• Promoting self-management strategies that empower patients to take an active role in their own recovery process.

• Assisting patients in adhering to prescribed medical treatments by offering coping strategies, especially when dealing with difficult side effects from medications.

• Engaging in regular consultations with clinic management and other team members to evaluate program outcomes and enhance service delivery.
