Position Overview

Lifepoint Physical Therapy is seeking a dedicated and experienced Full-Time Physical Therapist (OPT) to join our home care team in Bonita Springs, Florida. Work Environment

This position is primarily onsite, involving travel to different patient homes in Bonita Springs and surrounding areas. Key Responsibilities

• Conduct comprehensive patient assessments to evaluate mobility, strength, and functional capabilities.
• Develop individualized treatment and discharge plans based on assessment findings and patient goals.
• Implement therapeutic interventions, including manual therapy techniques, therapeutic exercise programs, and assistive device training.
• Maintain accurate patient records, detailing evaluations, treatment progress, and outcomes, while adhering to local and federal regulations.
• Collaborate effectively with interdisciplinary teams, including physicians, nursing staff, and other therapists, to ensure cohesive patient care.
• Engage in community outreach activities to promote physical therapy services and educate the public on health-related topics.
• Actively participate in staff meetings and ongoing professional development through continuing education and in-services.
• Supervise and mentor support staff, including physical therapy aides and interns, ensuring quality care delivery.

Patient Population / Client Focus

This role involves working with a diverse patient population, including individuals of all ages who may experience various physical impairments such as post-surgical recovery, chronic pain, neurological conditions, and orthopedic injuries. You will provide compassionate care tailored to each patient's specific circumstances, often developing strong rapport with patients and their families, significantly impacting their rehabilitation journey.

Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited institution is required.
• Current state licensure in Physical Therapy is mandatory.
• Strong verbal and written communication skills are essential for effective patient care and documentation.
• Proficiency in electronic medical records (EMR) and basic computer skills are necessary for maintaining accurate and timely documentation.

Preferred Experience

• A minimum of two years of experience in a physical therapy setting is preferred, with home health experience being a plus.
• Experience in geriatric care and knowledge of age-related conditions and rehabilitation techniques.

Compensation and Benefits

This position offers a competitive salary range of USD 100,000 to 120,000 annually, commensurate with experience.
