Position Overview

OnTrack Physical Therapy & Wellness is seeking a dedicated and skilled Orthopedic Physical Therapist to join our team in Bonita Springs, Florida. This full-time position offers a unique opportunity to provide home care services to patients in the orthopedic outpatient unit. Work Environment

This role is primarily dedicated to providing onsite care in patients' homes across the Bonita Springs area, making it a rewarding experience as you engage directly with clients in a comfortable and familiar environment. Our collaborative team atmosphere fosters continuous support and ensures that you are equipped with the necessary tools and resources to thrive. You will work Monday through Friday, from 8 AM to 5 PM, allowing for a balanced professional and personal life.

Key Responsibilities

• Conduct thorough evaluations of patients' physical capabilities, including history assessments and specific examinations, to develop individualized treatment plans.
• Implement treatment protocols based on patient evaluations and continuously monitor their effectiveness, making necessary adjustments to treatment plans as needed.
• Recognize and document changes in patients' conditions that may warrant modifications in treatment approaches, ensuring timely consultation with other healthcare professionals when necessary.
• Maintain up-to-date, accurate documentation reflecting patient care activities, including initial evaluations, daily encounter notes, progress summaries, and discharge evaluations.
• Prepare and execute comprehensive discharge plans that facilitate smooth transitions to ongoing care or independence.
• Communicate openly with co-workers, physicians, patients, and families to promote a collaborative approach to patient care.
• Participate in facility meetings, contributing valuable insights and experiences to enhance clinical practices.

Patient Population / Client Focus

Your focus will primarily be on orthopedic patients who require rehabilitation post-injury, surgery, or as part of a management plan for chronic conditions. These may include individuals recovering from joint replacements, sports injuries, or degenerative conditions. By providing hands-on care and tailored exercises, you will directly impact their journey towards recovery.

Qualifications

• A Doctorate in Physical Therapy (DPT) is required.
• A valid physical therapy license in the state of Florida is essential.
• Current certification in Basic Life Support (BLS) for Healthcare Providers from the American Heart Association.

Preferred Experience

While a minimum of two years of clinical experience as a physical therapist is required, additional experience in orthopedic settings or home care environments is preferred. Proficiency in adapting treatment plans to fit the unique needs of patients in their home environments is highly valued.

Compensation and Benefits
