Work Environment

This Physical Therapist position is entirely onsite, providing care directly within the homes of our clients in Bonita Springs and surrounding areas. Key Responsibilities

• Conduct comprehensive assessments of patients' abilities and limitations to develop personalized treatment plans aimed at improving function and mobility.

• Implement evidence-based treatment protocols and monitor progress, adjusting care plans as necessary based on patient feedback and performance.

• Document patient evaluations, treatment plans, and progress in a timely and clear manner to ensure continuity of care and adherence to regulatory requirements.

• Maintaining compliance with state, local, and federal regulations while actively engaging in ongoing education and training opportunities.

• Collaborate with multidisciplinary teams and other healthcare professionals to provide integrated care and address any barriers that may hinder patient access to services.

• Engage in discharge planning, ensuring patients have a clear roadmap for continued care and resources available for their rehabilitation journey.

• Actively participate in team meetings and clinical discussions, contributing to a healthy culture of learning and improvement within the organization.

Patient Population / Client Focus

In this role, you will work closely with diverse populations dealing with a range of conditions, from post-surgical recovery to chronic illness management. Your expertise will be fundamental in helping these patients regain strength and independence, alleviating pain and enhancing overall quality of life. As a home care provider, your interactions will be personalized, enabling you to build trust and rapport with each individual during therapeutic sessions.

Qualifications

• Doctorate in Physical Therapy (DPT) from an accredited program.

• A valid state license to practice physical therapy in Florida.

• Minimum of two years of relevant clinical experience, including working in home care settings.

• Current CPR certification is required.

• Exceptional interpersonal communication skills, with the ability to develop strong relationships with patients and their families.

Preferred Experience

• Experience with digital health solutions or app-based training is highly desirable.

• Prior engagement with patient education initiatives or support programs focused on home therapies is advantageous.

Compensation and Benefits
