Key Responsibilities\n

• Conduct comprehensive assessments to identify the unique needs of each patient, creating tailored therapeutic plans that address their concerns effectively.

• Facilitate communication between team members, family members, and other stakeholders regarding the patient's progress and treatment goals.

• Develop and manage a tentative schedule for therapy sessions, home visits, and interdisciplinary team meetings to monitor the patient's progress consistently.

• Coordinate home visits for in-depth observation, conducting chart reviews, and staff interviews to ensure data-driven treatment approaches.
\n
• Ensure the effective implementation of therapeutic plans and monitor their fidelity through regular observation and feedback.
\n
• Participate actively in Interdisciplinary Team Meetings to discuss ongoing cases, share insights, and contribute to holistic patient care strategies.

• Document and complete monthly and quarterly progress notes, reflecting the patient's journey and any necessary adjustments to care plans.
\n
• Organize and provide training sessions to enhance team members' skills in implementing program plans and maintaining proper documentation practices.

• Conduct follow-up interviews and observations, ensuring continued support for patients as they navigate their mental health journeys.

The therapist will work with a diverse range of patients, including adults and youth dealing with various mental health challenges. Qualifications\n
