Key Responsibilities\n

• Conduct comprehensive assessments of patients to tailor treatment plans that address their unique mental health needs.
\n
• Maintain clear channels of communication with supervisory staff, direct care team members, families, guardians, and other stakeholders to discuss patient progress and treatment adjustments.
\n
• Develop and manage a schedule of appointments, home visits, and follow-up observations to support ongoing assessment and care.
\n
• Coordinate home visits as necessary to ensure comprehensive reviews of patient interactions and compliance with prescribed care plans.
\n
• Ensure the program plan is implemented effectively and adjusted as necessary to meet the needs of patients.
\n
• Participate actively in Interdisciplinary Team Meetings and other essential forums to discuss patient care strategies and advancements.
\n
• Document patient progress through detailed monthly and quarterly notes to track outcomes and inform future care strategies.
\n
• Organize and conduct training for staff members to ensure consistency in implementing care strategies and documentation practices.
\n
• Provide continuous support through follow-up visits and observe patients in various environments to adapt care plans as needed.

Patient Population / Client Focus\n

In this role, you will work with a diverse patient population, including both adults and youth facing various mental health challenges. This includes individuals dealing with trauma, substance misuse, family dynamics, and other psychological needs. You will utilize evidence-based therapeutic modalities and culturally sensitive practices to address the complexities of each patient's circumstances and ensure effective treatment outcomes.

Qualifications\n

• A Master’s or Doctorate degree in a relevant field such as psychology, social work, or counseling is required.
\n
• Possession of an active and unrestricted state license as a Licensed Marriage and Family Therapist (LMFT), Licensed Professional Clinical Counselor (LPCC), or Licensed Clinical Social Worker (LCSW).

• A minimum of 2 years of clinical counseling experience is essential, ideally within a mental health or healthcare context.
\n
• Experience or specialization in family therapy, couples therapy, substance misuse treatment, or trauma-informed care is highly desirable.

• Demonstrated understanding and adherence to legal and ethical standards within the field of mental health counseling.
\n
• Proven ability to collaborate effectively within a multidisciplinary healthcare team.

• Salary range of USD 80,000 – 104,000 annually, commensurate with experience and caseload.
