Key Responsibilities\n

• Conduct comprehensive and ongoing assessments to accurately address the needs and goals of your patients.

• Maintain effective communication with supervisors, direct care staff, family members, and guardians to monitor and report the progress of each individual.

• Provide a detailed schedule of upcoming appointments, home visits, and supervision meetings to ensure organized care delivery.

• Coordinate home visits for in-depth observations, chart reviews, and staff interviews, ensuring continuous evaluation and support of each case.

• Implement program plans with diligence, ensuring that therapeutic interventions are executed correctly and effectively.

• Engage in Interdisciplinary Team Meetings to collaborate with healthcare professionals, share insights, and enhance patient care strategies.

• Complete detailed monthly and quarterly progress notes to track improvements and facilitate ongoing patient support.

Qualifications\n
