Key Responsibilities\n

• Conduct ongoing assessments of patients' mental health needs, utilizing evidence-based methods to create and implement individualized treatment plans.

• Maintain clear communication with supervisors, direct care staff, family members, and guardians about patient progress to ensure holistic care and continuity of services.

• Provide a tentative schedule of appointments, home visits, and observational activities to supervisors, ensuring all activities align with treatment goals.

• Organize and conduct home visits for in-home assessments, therapy sessions, and data gathering to monitor patient progress effectively.
\n
• Collaborate with interdisciplinary teams by participating in meetings, sharing insights, and developing coordinated care strategies for each patient.

• Compile and complete detailed monthly and quarterly progress notes that reflect patient responses to treatment and any necessary adjustments to care plans.
\n
• Facilitate training sessions for staff to ensure they are well-equipped to implement therapeutic programs and documentation accurately.

• Support individuals with follow-up interviews and ongoing home visits to address any emerging needs and provide consistent therapeutic support.

Patient Population / Client Focus\n

As a Licensed Mental Health Therapist at Advocate Aurora, you will work with a diverse range of patient populations, including both adults and youth experiencing various mental health conditions. Qualifications\n

• A Master’s or Doctorate degree in a relevant field such as Counseling, Social Work, or Psychology is required.

• Active and unrestricted licensure in California as a Licensed Marriage and Family Therapist (LMFT), Licensed Professional Clinical Counselor (LPCC), or Licensed Clinical Social Worker (LCSW).

• A minimum of 2 years of counseling experience in a clinical setting, demonstrating a commitment to excellence in mental health care.

• In-depth knowledge of legal and ethical standards in mental health practice.

• Proven ability to work collaboratively within a healthcare team, effectively communicating and coordinating care across disciplines.
