Key Responsibilities\n

• Psychosocial Assessment: Conduct comprehensive assessments to identify psychosocial factors impacting treatment outcomes, integrating knowledge of human behavior, ethical considerations, and legal obligations.

• Clinical Interventions: Render supportive counseling to individuals and families, facilitating linkages to community resources and aiding in complex decision-making, such as end-of-life care discussions and advance healthcare directives.

• Caseload Management: Efficiently manage a diverse clinical caseload, identifying and addressing high-risk situations, coordinating with case managers during discharge planning, and ensuring continuity of care for patients.

• Documentation: Maintain accurate and timely documentation in accordance with organizational policies including social services progress notes and interdisciplinary plans of care.

• Training and In-Service: Provide educational sessions for hospital staff regarding psychosocial aspects of patient care, including participation in initiatives such as Lunch and Learns.

• Quality Improvement: Engage in monitoring and analysis of clinical performance indicators, contributing to organizational efforts focused on enhancing patient care quality.

• Supervision: Supervise graduate social work students and MSW employees, providing guidance and support to enhance their clinical skills while adhering to all ethical standards.

• Other Duties: Participate in relevant social work events and activities, supporting further education and outreach initiatives.
