Key Responsibilities\n

• Conduct thorough bio-psychosocial assessments for patients faced with terminal illnesses, integrating considerations of mental health, substance abuse, and other medical needs.

• Develop and implement individualized treatment plans, actively engaging with patients and their families to address emotional and psychological issues during the hospice process.

• Facilitate individual and group counseling sessions, supporting patients and their families through grief, fear, and adjustment issues.

• Perform risk assessments to identify patients in crisis and provide immediate crisis intervention as needed.

• Serve as a resource and liaison for families, connecting them with relevant community services, including medication assistance programs and local support groups.

• Collaborate with healthcare teams to improve patient outcomes and quality of care through data-driven approaches and quality improvement initiatives.

Your primary focus will be on patients in hospice care, including individuals from diverse age groups and backgrounds. Qualifications\n

Compensation and Benefits\n
