This position will be performed remotely, allowing for flexibility while maintaining a connection with clinical teams and patients. In this role, you will collaborate with fellow social workers, psychologists, physicians, and case managers from various locations. Regular video conferencing and digital communication will be utilized to facilitate patient care discussions and interdisciplinary meetings.

Key Responsibilities\n

• Conduct timely and thorough psychosocial assessments that leverage your knowledge of human behavior and psychosocial factors impacting treatment outcomes. You will assess patient situations, including identifying high-risk circumstances and addressing potential ethical or legal concerns.

• Manage therapeutic clinical interventions, which include providing supportive counseling to patients and families, linking them to community resources, and facilitating family conferences to navigate challenging situations such as hospice or advanced care directives.

• Effectively balance and manage a clinical caseload, ensuring that high-risk admissions and ongoing patient care are prioritized. You will collaborate closely with case managers to coordinate discharge planning and clinical follow-ups.

• Uphold legal and ethical standards throughout your practice, addressing issues like suspected abuse or neglect, and ensuring patient rights are respected in all interactions.

• Maintain meticulous documentation per institutional policies, including progress notes and interdisciplinary care plans, to foster transparency and accountability in patient care.

• Provide in-service training for hospital staff, sharing insights regarding psychosocial issues that affect patient care, through workshops and interdepartmental meetings.
\n
• Compile and submit timely social service reports, including case assignments and compliance documentation, to support operational efficiency.

• Contribute to quality improvement initiatives by actively participating in monitoring and analyzing care process and outcome indicators. This role plays a vital part in research and improvements aimed at elevating the standard of care.

• Actively participate in various social work initiatives, including organizing special events, leading support groups, and facilitating educational programs for patients and their families.

• Perform additional duties as assigned, which may include engagement in community outreach activities and interdisciplinary collaboration projects.

You will work with a varied patient demographic within our community health clinic, focusing on strengthening the emotional and social well-being of individuals facing medical challenges, especially those within a cancer care setting. Your work will directly support patients and families dealing with complex health issues, including chronic illnesses, mental health disorders, and end-of-life considerations.

Qualifications\n

• A Master's degree in Social Work from an accredited school.

• A minimum of 2 years of experience working in a medical setting, with at least 1 year focused in oncology or related cancer care environments.

• Strong written and verbal communication skills in English, coupled with exceptional analytical and problem-solving abilities.

• Proven ability to foster collaborative relationships with medical staff, patients, and their families.

Compensation and Benefits\n

The base salary for this position ranges from $120,000 to $130,000 annually, commensurate with experience and qualifications.
