This role is fully remote, allowing for flexibility while ensuring our LCSW remains connected to the hospice center’s interdisciplinary team. Key Responsibilities\n

• Directly assess and support clients experiencing symptoms of major mental illness and emotional disorders, both in the community and via teletherapy.

• Create and implement individualized integrated care plans that reflect clients' unique needs, ensuring coordination with other healthcare providers and family members.

• Conduct brief, evidence-based screenings and assessments to identify mental health conditions, following up with more extensive evaluations when necessary.

• Monitor and document client progress through routine assessments and therapy sessions, using electronic health record systems to ensure accurate and timely data entry.

• Collaborate with an interdisciplinary team to further the goals of treatment plans and improve overall patient outcomes.

• Facilitate individual, group, and family therapy sessions focused on recovery, coping strategies, and emotional health enhancement.

• Provide crisis intervention services as needed and advocate for clients' needs within healthcare and community resources.

• Participate in ongoing training and professional development initiatives to keep skills up to date and align with best practices in mental health.

Patient Population / Client Focus\n

Our clients include individuals with severe emotional disorders, such as depression, anxiety, and other mental health complications, often facing overlapping health and socio-economic challenges typical within hospice care. The LCSW will engage with a diverse population, often requiring a sensitivity to cultural differences and an understanding of the unique challenges individuals face in end-of-life scenarios.

Qualifications\n

• A Bachelor’s Degree in Social Work or a related field is required; a Master's degree is preferred.

• A valid Licensed Clinical Social Worker (LCSW) license in Pennsylvania is required.

• At least two years of relevant experience in social work, particularly in mental health or hospice settings.

Compensation and Benefits\n

We offer a competitive salary range of USD 70,000 to 85,000 annually, commensurate with experience.
