Key Responsibilities\n

As a Licensed Clinical Social Worker, you will be responsible for a range of essential functions, including but not limited to:

• Conducting comprehensive evaluations and psychosocial assessments to identify the needs and concerns of patients and their families.

• Participating actively in the development and implementation of Individualized Family Service Plans (IFSPs) to address the unique circumstances of each case.

• Monitoring and reporting outcomes and progress, ensuring effective communication with all members of the healthcare team.

• Providing emotional support, guidance, and education to families, helping them navigate challenges and enhancing the quality of care for their loved ones.

• Utilizing evidence-based practices and natural learning environment principles to create individualized strategies that promote development and emotional well-being.

• Adhering to all federal and state regulations to ensure compliance and the highest quality of care.

Patient Population / Client Focus\n

This role primarily supports patients in hospice care, many of whom are facing terminal illness or significant health challenges. You will work collaboratively with families, enhancing their understanding of the care process while providing crucial emotional support. The LCSW's work is integral to improving patients' quality of life during their final stages, ensuring comfort and dignity.

Qualifications\n

• A Master's Degree in Social Work (MSW) from an accredited institution or a Doctorate of Social Work (DSW).

• Current licensure as a Licensed Clinical Social Worker (LCSW) recognized by the Department of Public Health.

• A valid driver's license, and willingness to travel as needed to meet client needs.

• Strong aptitude for adapting to various learning styles and emotional needs of children and families.

• Proficient organization and time management skills, combined with the ability to work independently.

• Exceptional communication skills, both verbal and written.

Preferred Experience\n

• A minimum of one year of experience working with children within early intervention frameworks.

• Experience in hospice or palliative care settings is highly desirable.

Compensation and Benefits\n
