Key Responsibilities

• Evaluate, plan, and implement individualized treatment programs for patients referred by physicians, tailoring each plan to meet specific patient needs.
• Utilize a range of therapeutic techniques and modalities to facilitate recovery and enhance patients' physical function in a home setting.
• Document patient progress through detailed reports, maintaining compliance with legal and ethical standards.
• Work collaboratively with an interdisciplinary team, including physicians and other healthcare workers, to maximize treatment effectiveness.
• Educate and train patients and their caregivers on treatment plans and recovery strategies, fostering independence and self-management.
• Maintain excellent customer service standards, ensuring that each patient's experience reflects Oakbend's commitment to quality care.
• Meet utilization goals and contribute to administrative tasks, ensuring the smooth operation of our home health services.

Patient Population / Client Focus

Your role will involve working with a diverse patient population, including individuals recovering from orthopedic surgeries, pediatric patients with developmental challenges, neurologic conditions, and athletes needing sports rehabilitation. You will be instrumental in delivering specialized care that addresses the unique needs of each patient, ensuring they receive the attention and support required to achieve their rehabilitation goals.

Qualifications

• A Doctorate in Physical Therapy (DPT) from an accredited program.
• A valid state license for Physical Therapy in Virginia.
• Basic Life Support Certification from the American Heart Association.
• Proven written and verbal communication skills to effectively collaborate with team members and educate patients.
• Strong computer skills, including proficiency in word processing and documentation software.
• The ability to build rapport with patients and establish therapeutic alliances for improved outcomes.

Preferred Experience

While a minimum of 2 years of experience as a licensed physical therapist is required, candidates with a background in home health care or experience working with a variety of patient populations will be given preference. Compensation and Benefits
