Work Environment

This full-time position is based entirely onsite, delivering home care services to our patients. Your work environment will vary, as you will travel to clients' residences across Appomattox and surrounding areas. Our team consists of two experienced physical therapists and one physical therapist assistant, creating a supportive and collaborative atmosphere where team members work together to provide the best patient outcomes. You will utilize evidence-based practices and cutting-edge technologies in patient assessments and treatment plans, ensuring that every patient receives the highest standard of care.

Key Responsibilities

• Conduct thorough assessments to identify patient needs, health goals, and functional limitations.
• Create and implement individualized care plans that utilize a variety of therapeutic modalities, including manual therapy, exercise regimens, and advanced rehabilitation techniques.
• Provide treatment to 2-3 patients per hour, focusing on enhancing their mobility, strength, and overall physical health.
• Incorporate patient education into each session, teaching techniques for self-management and preventative care.
• Collaborate with other healthcare professionals, including physicians and specialists, to ensure comprehensive and coordinated care for each patient.
• Document all patient interactions and progress using electronic medical records (EMR) software such as WebPT, ensuring accurate and timely updates.
• Monitor and evaluate patient progress, adjusting treatment plans as needed to optimize recovery outcomes.
• Encourage patients through their recovery journey, motivating them to achieve their personal best during treatment.
• Document and archive all treatment services while keeping track of patient progress and advancements.

Patient Population / Client Focus

As a physical therapist at Elite Rehab, you will work predominantly with patients in their homes who may be recovering from surgery, managing chronic pain, or rehabilitating from musculoskeletal injuries. Our clients range from active individuals seeking to improve performance to seniors aiming to maintain independence through enhanced mobility. Each patient presents unique challenges, and your role is to create personalized strategies that facilitate their progress and comfort.

Qualifications

• A Doctor of Physical Therapy (DPT) degree from an accredited program.
• Current state physical therapist license, with no disciplinary actions on record.
• Strong manual therapy skills, demonstrating proficiency in techniques to improve patient outcomes.
• Familiarity with EMR systems, specifically WebPT or similar platforms.
• Exceptional interpersonal and communication skills, enabling effective collaboration with patients and healthcare teams.

Preferred Experience

• Minimum of 2 years of experience in a clinical setting, preferably in outpatient orthopedic care.
• Prior experience with home health therapy, understanding the nuances of providing care in a patient's home environment.
• Strong background in developing and implementing wellness regimens tailored to individual needs.

Compensation and Benefits
