Work Environment

This position is primarily onsite, focusing on home care therapy. You will work closely with patients in their residences, allowing for personalized therapeutic interventions that directly address individual health concerns in familiar surroundings. The typical work schedule is Monday to Friday, with hours from 8:00 am to 5:00 pm, providing a structured routine that fosters work-life balance.

Key Responsibilities

• Comply with state practice acts and regulatory standards governing physical therapy practice, ensuring adherence to legal and ethical guidelines.
• Conduct thorough evaluations of patients' physical capabilities and limitations, developing evidence-based treatment plans tailored to each individual's needs.
• Implement effective treatment protocols based on assessment findings, continually evaluating patient progress and modifying plans as necessary.
• Document patient care activities accurately and comprehensively, including initial evaluations, treatment plans, ongoing progress notes, and discharge summaries, while ensuring compliance with evolving healthcare regulations.
• Collaborate with other healthcare professionals to enhance patient care, addressing any changes in patients' conditions that may require additional support or intervention.
• Participate in the discharge planning process and provide follow-up care recommendations to support patients' continued recovery.
• Engage in facility meetings and contribute to the development of strategies aimed at removing barriers to access for therapy services.

Patient Population / Client Focus

As a Physical Therapist specializing in home care, you will work with a diverse patient population, including individuals recovering from surgery, those managing chronic diseases, and clients with mobility challenges. Your role will greatly influence their rehabilitation journey, empowering them to regain independence and improve their overall quality of life.

Qualifications

• A Doctorate of Physical Therapy (DPT) from an accredited physical therapy program.
• State licensure as a Physical Therapist (PT) issued by the Physical Therapy Examining Committee.
• A minimum of two years of outpatient orthopedic experience, demonstrating a strong foundation in addressing various musculoskeletal conditions.
• Current CPR certification to ensure patient safety and emergency preparedness.

Preferred Experience

Experience in home health care settings is highly regarded, as well as familiarity with electronic health record systems to maintain accurate patient documentation. Additional training or certifications in specialized areas of physical therapy would be advantageous and further enhance our treatment capabilities.

Compensation and Benefits

Your annual salary for this position will range from USD 100,000 to 120,000, commensurate with your experience and qualifications.
