Work Environment

You will be working onsite in a home care setting, which enables you to provide individualized treatment in a comfortable and familiar environment for our patients. Our work schedule is structured, running from Monday to Friday for 40 hours per week, allowing for a balanced work-life dynamic while meeting the clinical needs of our patients.

Key Responsibilities

• Conduct thorough evaluations and assessments of patients referred by physicians, establishing detailed treatment plans that align with personal goals and medical recommendations.
• Implement and modify treatment programs as necessary to ensure the highest level of care and optimal patient outcomes.
• Utilize evidence-based practices to create and deliver skilled therapeutic interventions, whether through manual therapy techniques, exercise, or modalities.
• Demonstrate exceptional customer service, ensuring that patients and their families feel supported and valued throughout their recovery journey.
• Collaborate closely with interdisciplinary teams to maximize patient outcomes, engaging in regular case reviews and treatment strategy discussions.
• Provide education and training to patients and their caregivers about exercises, techniques, and lifestyle adjustments to foster independence and recovery.
• Meet and exceed established utilization goals and contribute toward achieving the business objectives of the practice.

Patient Population / Client Focus

Our practice serves a diverse range of patients, including those with orthopedic injuries, pediatric conditions, neurological disorders, and sports-related injuries. As a Physical Therapist, you will be integral in assisting these individuals throughout various phases of their healing process, helping them regain functionality and improve their quality of life.

Qualifications

• A graduate of an APTA or CAPTE-approved physical therapy program with a Bachelor's degree in Physical Therapy.
• Possess an active State License in Physical Therapy and a current Basic Life Support Certification from the American Heart Association.
• Demonstrated strong written and verbal communication skills to effectively interact with patients and healthcare professionals.
• Proficient in word processing and general computer skills, essential for patient documentation and care coordination.
• A commitment to building strong therapeutic relationships with patients, enabling them to achieve and surpass their functional goals.
• A passion for positively connecting with people and making a difference in their lives through physical therapy.

Preferred Experience

• Prior experience in a home health or outpatient physical therapy setting is preferred, allowing for deep familiarity with hands-on therapeutic techniques.
• Experience managing a caseload with an emphasis on orthopedic, pediatric, and neurologic conditions enhances your capability to meet diverse patient needs.

Compensation and Benefits
