Key Responsibilities

• Conduct comprehensive evaluations for patients referred by physicians, developing individualized treatment plans tailored to their specific needs and conditions.
• Implement evidence-based therapeutic techniques and treatment modalities to support recovery, rehabilitation, and functional improvement within the comfort of the patient's home.
• Maintain effective communication with patients, caregivers, and medical personnel to ensure coordinated care and optimal patient outcomes.
• Provide education and training to patients and their families, empowering them regarding their rehabilitation plans and promoting adherence to therapy.
• Utilize specialized tools and systems for documentation and tracking of patient progress, ensuring compliance with care standards and responding to evolving patient needs.
• Achieve operational goals related to patient attendance, utilization metrics, and overall service effectiveness.

Patient Population / Client Focus

Qualifications

• A Bachelor's degree in Physical Therapy from an accredited program (APTA or CAPTE-approved).

Compensation and Benefits
