Work Environment

This position operates in the familiar comfort of the patient's home. As a Physical Therapist at Active Life Rehab, you will work within the community, allowing you to establish closer relationships with your patients and adapt your treatment methods to their specific living environments. The flexible hours enable you to align your professional commitments with personal life, establishing a healthy work-life balance. The general schedule runs from Monday to Friday, 9 AM to 5 PM.

Key Responsibilities

• Conduct detailed physical assessments to evaluate patients' physical and functional abilities, ensuring comprehensive diagnostic insights that inform treatment plans.
• Create individualized treatment strategies tailored to each patient's unique goals, challenges, and health conditions, focusing on optimizing recovery.
• Implement hands-on therapeutic techniques and exercises, and apply various modalities, including heat therapy, electrical stimulation, and manual therapy to alleviate pain and restore function.
• Educate patients and their families on effective strategies for maintaining mobility and preventing injury, enhancing their overall health education.
• Continuously monitor patient progress, documenting advancements and setbacks, and adjusting treatment plans as necessary to optimize patient care.
• Collaborate with interdisciplinary teams, including physicians, occupational therapists, and nursing staff, to ensure integrated and holistic patient care.
• Maintain meticulous records of treatment protocols, outcomes, and patient interactions to comply with healthcare regulations and to inform future treatment decisions.

Patient Population / Client Focus

Our patient population is diverse, encompassing individuals recovering from surgeries, managing chronic pain, and those experiencing mobility issues due to age-related conditions or injuries. We treat conditions such as back and neck pain, osteoarthritis, tendonitis, sprains, strains, and the rehabilitation needs following surgical interventions. As a therapist, you will be instrumental in guiding patients through their rehabilitation journey, fostering an environment of healing and support.

Qualifications

• A Bachelor's or Master's degree in Physical Therapy from an accredited program.
• Current licensure to practice as a Physical Therapist in the state of New Jersey.
• Active board certification from the American Physical Therapy Association (APTA).
• A minimum of five years of clinical experience in a physical therapy setting, demonstrating a comprehensive understanding of varied patient needs.
• Proficiency in contemporary physical therapy techniques and therapeutic modalities.
• Strong analytical skills to assess rehabilitation needs and devise effective treatment strategies.
• Exceptional communication and interpersonal abilities to build rapport with patients, families, and multidisciplinary healthcare teams.

Preferred Experience

• Experience in home health care settings is a significant plus, as it involves adapting treatment to diverse environments and patient needs.

Compensation and Benefits
