Key Responsibilities\n

• Communicate closely with medical professionals, including physicians, to relay patient progress, needs, and any emergent issues impacting client care.

• Maintain meticulous, timely documentation of clinical notes and patient progress records in accordance with our department’s protocols and electronic medical record systems.

• Provide immediate intervention and support during crisis situations, ensuring the safety and well-being of clients while addressing their mental health needs.

• Regularly assess clients’ progress towards their treatment goals and adapt therapeutic plans using evidence-based practices.

• Engage with clients’ families and support systems, facilitating communication that enhances treatment planning and discharge processes.

Patient Population / Client Focus\n

This position primarily involves working with clients in hospice care settings, where emotional support and therapeutic interventions play a vital role in achieving quality of life during end-of-life stages. You will address a range of psychological and emotional challenges, including coping with grief, loss, and trauma, while guiding clients and their families through the complexities of transitioning care.

Qualifications\n

• Bachelor’s degree in Social Work, Counseling, Psychology, or a related field is required.

• Must possess an active state license as an LICSW or an equivalent designation.
\n
• A minimum of three years of experience providing clinical services in outpatient settings, with a strong focus on geriatrics and hospice care preferred.

• Ability to work independently with a high degree of self-motivation, organization, and time management.

• Proficient in using technology for word processing, email communication, spreadsheet analysis, and electronic health record systems.

Preferred Experience\n

While not mandatory, experience with hospice care, palliative therapies, or grief counseling will be highly advantageous. Familiarity with telehealth platforms and methodologies of care coordination within interdisciplinary teams will also bolster your candidacy.

Compensation and Benefits\n

The salary for this position ranges from $100,000 to $105,000 annually, commensurate with experience and qualifications.
