Key Responsibilities\n

• Utilize your education, training, and clinical experience to conduct thorough psychosocial assessments and develop independent diagnoses for patients.

• Collaborate with patients to create individualized treatment plans that align with their unique needs, goals, and preferences.

• Implement a range of evidence-based behavioral interventions, including techniques such as motivational interviewing, solution-focused treatment, and behavioral activation, to enhance therapeutic outcomes.

• Engage in multidisciplinary coordination with other healthcare professionals including psychologists and primary care providers, fostering a holistic approach to patient care.

• Monitor patient progress through systematic tracking of treatment responses and outcomes, using both in-person and virtual check-ins.

• Assist in the management of psychotropic medications by collaborating with medical providers to ensure adherence, monitor side effects, and assess overall treatment efficacy.

Patient Population / Client Focus\n

As an LCSW at our clinic, you will work with a diverse client base that includes individuals experiencing emotional and psychological distress related to various life transitions, chronic illnesses, and grievous situations often encountered in hospice settings. Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• A current and unrestricted license to practice as a Licensed Clinical Social Worker (LCSW) in California.
