Key Responsibilities\n

• Independently manage a caseload of patients referred from schools, performing evaluations that consider emotional, financial, cultural, and social factors impacting their well-being.

• Develop and implement individualized treatment plans to address complex psychosocial issues, ensuring a seamless transition through the continuum of care.
\n
• Provide crisis intervention and coordinate the healthcare team to address protective issues and life care decisions for high-risk patients.
\n
• Foster collaboration with case managers and other healthcare professionals to ensure comprehensive education, resources, and referrals are provided to patients and their families.
\n
• Supervise and mentor staff, including Social Work interns and volunteers, while promoting effective practice standards within the department.
\n
• Engage with community resources and agencies to optimize patient care and prevent hospital readmission, ensuring patients’ psychosocial needs are met.
\n
• Conduct timely assessments of psychosocial needs to prevent delays in treatment plans, aiming for overall enhanced patient satisfaction and outcomes.

• Participate in facility consultations and specialty team rounds to share insights and strategies for improving patient care coordination.
\n
• Stay available for on-call situations as needed, supporting medical teams and crisis issues when they arise.

This role primarily focuses on students in the school setting, requiring a deep understanding of the unique challenges they face. You will work closely with children and adolescents, as well as their families, to tackle behavioral, emotional, and developmental issues. Your role is pivotal in helping them navigate various barriers while they receive educational opportunities and mental health support.

Qualifications\n

• A valid and active Licensed Clinical Social Worker (LCSW) license.

• A Bachelor’s Degree in Social Work or a related field.

• A minimum of two years of professional experience as a Social Worker.

Compensation and Benefits\n

This full-time position offers a competitive annual salary ranging from USD 120,000 to 130,000, commensurate with experience and qualifications.
