Work Environment

This role is primarily onsite in a home care setting, enabling our therapists to engage directly with patients in their own environments. Key Responsibilities

• Conduct in-home evaluations to assess patient conditions, recovering from injuries, surgeries, or chronic illnesses.
• Design and implement individualized treatment plans that may include modalities such as therapeutic exercises, manual therapy techniques, and the use of assistive devices to promote healing and mobility.
• Monitor patient progress closely by tracking and documenting treatments, outcomes, and any changes in patient condition over time.
• Adjust treatment plans as necessary based on reevaluations and set new therapeutic goals to ensure continuity of care.
• Educate patients, their families, and caregivers on treatment regimens, proper usage of equipment, and effective home exercises to support ongoing recovery.
• Participate in continuous learning by attending workshops, reviewing relevant literature, and engaging with professional networks to maintain current knowledge and skills.

Patient Population / Client Focus

Our clientele includes patients of all ages dealing with a variety of physical issues, from post-surgical rehabilitation to managing chronic pain and improving mobility for conditions such as arthritis or stroke recovery. Qualifications

• A Doctorate in Physical Therapy (DPT) is required.
• A valid state license in Physical Therapy is essential for practice.

• Demonstrated flexibility and adaptability in a home care setting.

Preferred Experience

Compensation and Benefits
