Work Environment\n

This position offers a remote work setting, allowing you to connect with clients and their families from the comfort of your own home while visiting hospice centers as required. As part of our team, you will have the flexibility to set your schedule primarily during regular business hours, with some occasional evening or weekend hours based on client needs. This role also requires on-call availability to ensure timely support, demonstrating our commitment to client care and community resources.

Key Responsibilities\n

• Conduct comprehensive assessments of clients' psychosocial needs, focusing on emotional, mental, and social dimensions.

• Collaborate with clients and their families to create and implement tailored care plans that respect individual preferences and goals.

• Provide individual and family counseling sessions, guiding seniors and their loved ones through difficult transitions and emotional challenges.

• Advocate for clients’ rights, helping them access necessary healthcare services, community resources, and support systems.

• Work in coordination with an interdisciplinary team of healthcare professionals, including nurses and caregivers, to ensure seamless and high-quality care delivery.

• Regularly visit clients to monitor their progress and well-being, addressing any concerns or adjustments needed regarding their care plans.

• Facilitate family meetings to improve communication, resolve conflicts, and enhance the collective caregiving experience.

• Provide education to clients and families about community resources, support groups, and programs that enhance their quality of life.

• Maintain accurate documentation of assessments, care plans, and client interactions in accordance with regulatory standards and best practices.

Your primary focus will be on seniors receiving hospice care, many of whom may face progressive illnesses, emotional distress, or psychosocial challenges related to their health conditions. By addressing their unique needs, you will play a significant role in improving their quality of life, helping them and their families find peace and support during a vulnerable time.

Qualifications\n

• Master's degree in Social Work from an accredited institution is required.

• Current and valid state licensure as a Licensed Master Social Worker (LMSW).

• A minimum of two years of experience in a social work capacity, ideally in geriatric or hospice settings.

• In-depth knowledge of social work principles, aging-related issues, and community resource navigation.

• Strong assessment, counseling, and intervention skills tailored to the elderly population.

Preferred Experience\n

Compensation and Benefits\n
