Work Environment

This role requires onsite presence, primarily visiting patients in their homes across the Englewood area. As a home health physical therapist, you will develop close relationships with patients and their families, working in a variety of settings that extend beyond the walls of a traditional rehabilitation facility.

Key Responsibilities

• Conduct comprehensive evaluations to assess patients' physical abilities, identify limitations, and establish therapeutic needs through detailed interviews, physical assessments, and observation techniques.
• Formulate individualized treatment plans that are tailored to each patient's goals, ensuring that they align with clinical best practices and rehabilitation standards.
• Implement hands-on therapy interventions, which may include therapeutic exercises, manual therapy, gait training, and balance exercises to enhance mobility and functionality.
• Educate patients and their families about the rehabilitation process, strategies for pain management, and exercises they can perform between therapy sessions to promote healing and wellness.
• Maintain accurate documentation of patient progress, including evaluations, treatment plans, and progress notes, in accordance with company policies, insurance standards, and regulatory requirements.
• Continuously monitor patient progress and adapt treatment plans as necessary, collaborating with patients and healthcare professionals to optimize recovery outcomes.

Patient Population / Client Focus

Your role will focus on managing a diverse patient population that may include seniors recovering from surgeries, adults with chronic conditions, or individuals rehabilitating from acute injuries. You will play a critical part in their recovery journey by providing high-quality care tailored to their specific needs and challenges, fostering an environment of support and encouragement.

Qualifications

• Completion of a Doctoral or Master's Degree in Physical Therapy from an accredited program.
• An active Physical Therapy license in the state of New Jersey is mandatory.
• A minimum of 2 years of clinical experience in physical therapy, preferably with exposure to home health or outpatient care settings.
• Exceptional clinical assessment skills and proficiency in manual therapy techniques, with a well-rounded understanding of rehabilitation approaches.

Preferred Experience

• CPR & First Aid certification is preferred; additional certifications in areas such as geriatrics or neurological rehabilitation are advantageous.
• Proficiency with electronic health record (EHR) systems for meticulous documentation and patient tracking.
• A valid driver's license and reliable transportation are required for travel to various patient locations.

Compensation and Benefits
