Key Responsibilities\n

• Conduct in-depth mental health assessments for patients receiving hospice care, exploring their backgrounds, emotional states, and specific end-of-life needs.

• Develop tailored treatment plans in partnership with patients and their families, focusing on addressing mental, emotional, and spiritual challenges that arise during hospice care.

• Utilize evidence-based therapeutic modalities including cognitive-behavioral therapy (CBT) and supportive counseling to assist patients in coping with their circumstances.

• Facilitate individual and group therapy sessions, empowering patients and their families to express their feelings and concerns in a safe environment.

• Provide crisis intervention as needed, employing de-escalation strategies and making referrals for more intensive psychological support if required.

• Educate patients and families about available mental health resources, ensuring they are informed about treatment options and support services.

• Maintain detailed and accurate patient documentation, ensuring compliance with all legal and ethical standards for mental healthcare.

• Participate in virtual case consultations and team meetings regularly to share insights and coordinate multidisciplinary approaches to patient care.

The primary focus of this role is on individuals in hospice care, including patients facing terminal illnesses. Qualifications\n

• A Master's degree in Social Work (MSW) from an accredited institution.

• Current LCSW licensure in the state of New York.

Preferred Experience\n

Compensation and Benefits\n

The compensation for this position ranges from USD 80,000 to 100,000 annually, based on experience and qualifications.
