Work Environment

This role will take place primarily in a home care setting, where you will meet clients in their homes, providing deeply personalized care tailored to their specific needs. Your work schedule will generally be from 9:00 a.m. to 6:00 p.m., with occasional evening clinics extending until 8:00 p.m. Participation in weekend community outreach events and health fairs is also expected, underscoring our commitment to community health advocacy. This role offers an average workload of approximately 40 hours per week.

Key Responsibilities

• Perform thorough health assessments and screenings, evaluating both acute and chronic health conditions within the community.
• Diagnose and manage patient care, focusing on preventive measures and health promotion strategies.
• Embrace a collaborative approach by working closely with local organizations to develop and implement effective health education programs.
• Provide culturally competent and sensitive care while advocating for patients' rights and access to necessary healthcare services.
• Coordinate comprehensive referrals and ensure effective follow-up care, fostering continuity and quality in healthcare delivery.
• Maintain accurate and detailed medical records to comply with all regulatory standards, enhancing the overall quality of patient care.
• Actively engage in community outreach initiatives, presenting health information and providing screenings at local fairs and events.
• Stay informed about emerging health trends and evidence-based practices to ensure our care approaches are current and effective.

Patient Population / Client Focus

Your role as a Family Nurse Practitioner at Biotech Advance Medical Center will involve working primarily with underserved individuals and families facing various health challenges. You will focus on addressing social determinants of health and leveraging community resources to provide comprehensive support. By understanding the unique needs of each patient, you will help foster improved health outcomes and build lasting relationships within the community.

Qualifications

• Current certification as a Nurse Practitioner is required.
• An active and unrestricted nursing license in the state of Maryland.
• A Master’s degree in nursing from an accredited educational institution.
• A minimum of 3-5 years of clinical experience, ideally within community health or public health fields.
• Exceptional interpersonal and communication skills, particularly in engaging diverse populations.

Preferred Experience

• Experience with electronic health record (EHR) systems for maintaining patient records.
• Knowledge of community resources and local healthcare programs.
• Track record of involvement in health advocacy and education initiatives.
• Familiarity with the social complexities facing low-income or uninsured populations.

Compensation and Benefits

The compensation package for this role includes a competitive salary range of $180,000 to $200,000 per year, commensurate with experience and qualifications.
