Key Responsibilities\n

• Conduct initial assessments and ongoing evaluations of assigned clients, implementing evidence-based treatment plans tailored to individual needs.

• Collaborate with multidisciplinary teams to ensure integrated care and continuity of services, participating actively in team meetings and case conferences.

• Create, maintain, and monitor individualized treatment plans, ensuring they are regularly updated based on client progress and feedback.

• Provide therapeutic interventions—including individual, group, and family therapy—to address symptoms of major mental illness and emotional disorders.

• Assist clients in crisis intervention situations, employing relevant clinical skills to stabilize emotional distress and ensure safety.

• Document all clinical activities accurately and in a timely manner, meeting organizational guidelines for reporting and data submission.

• Participate in quality improvement initiatives within the agency to enhance service delivery and patient outcomes.
