Key Responsibilities\n

• Conduct thorough assessments and develop individualized treatment plans tailored to meet the needs of each patient, incorporating crisis intervention strategies when necessary.

• Provide timely and accurate documentation to meet regulatory standards and ensure continuity of care.

• Utilize evidence-based screening tools in collaboration with the clinical team to accurately assess patient needs and progress.

• Work as part of an interdisciplinary team to create and implement positive behavior support plans that align with best practice methodologies.

• Engage actively in various licensing and accreditation processes to uphold the quality and integrity of our services.

• Foster relationships with relevant service systems, state agencies, and community partners to optimize patient care and access to resources.

• Maintain effective communication lines to ensure compliance with contract standards and promote the agency’s mission within the community.

• Develop and maintain networks with local, state, and federal organizations to identify funding opportunities and collaborative partnerships.

Patient Population / Client Focus\n

In this role, you will work with a diverse set of clients, including individuals dealing with mental health disorders, those in transitional periods of their lives, or patients recovering from substance use disorders. Your expertise will help guide these clients toward recovery and empowerment, creating a nurturing environment where they can thrive.

Qualifications\n

• A Master’s degree in a behavioral health field is required.

• Must hold an independent LMHC license as designated by the state board.

Compensation and Benefits\n
