Work Environment

Our hospice center operates primarily in a remote format, fostering a flexible and collaborative virtual environment. Our team utilizes advanced telehealth technology, ensuring that care remains connected and comprehensive despite geographic distance. You will work remotely while collaborating with nurses, physicians, and other healthcare professionals to deliver holistic support to clients and their families.

Key Responsibilities

• Design and implement individualized treatment plans that address mental health concerns, utilizing evidence-based practices and therapeutic techniques tailored to each client's needs.
• Conduct thorough assessments and evaluations to identify the social and emotional factors affecting clients and their families, ensuring an integrated approach to care.
• Serve as an advocate for clients, connecting them with necessary resources, community services, and additional support systems to foster comprehensive personal development.
• Engage in regular team meetings and case consultations with interdisciplinary colleagues, promoting collaborative strategies in client care.
• Utilize digital platforms and virtual counseling tools to facilitate remote therapy sessions, ensuring continuity of care while maintaining confidentiality and professional standards.
• Document all client interactions, assessments, and progress notes clearly and accurately, adhering to organizational policies and state regulations.

Patient Population / Client Focus

This role focuses on providing mental health support to clients facing terminal illnesses and end-of-life issues, along with their families. You will work with a diverse population that includes individuals from various backgrounds and cultural contexts, requiring sensitivity and awareness of their unique challenges and needs.

Qualifications

• A Master’s degree in Social Work from an accredited program is required.
• A valid License as an Independent Clinical Social Worker (LICSW) in Washington State, in good standing.
• Demonstrable experience (minimum of two years) in clinical social work, particularly in mental health support and client placement.
• Exceptional communication skills, both verbal and written, with a proven ability to build relationships with clients and collaborate effectively with a multidisciplinary team.
• Proficiency with technology and experience using telehealth platforms for virtual therapy sessions.

Preferred Experience

• Previous experience working in a hospice or end-of-life care setting is highly desirable.
• Knowledge of grief counseling and support interventions is a plus, as is familiarity with community mental health resources.
• Experience in conducting group sessions or workshops can enhance the support offered to clients.

Compensation and Benefits
