Registered Nurse (RN) - Home Health - $75K-100K per year

Position Overview Registered Nurse (RN) for Home Health – ongoing, 40 hours per week, 8‑hour shifts, Staff employment, located in Southview, Pennsylvania. Key Responsibilities Deliver skilled, patient‑centered care in home health setting, collaborating with interdisciplinary team. Perform patient assessments, medication management, and care planning. Educate and empower patients and families on disease management and treatment plans. Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation. Work independently while coordinating with team; travel to patient homes within assigned territory. Qualifications Active Registered Nurse license in state of work. Minimum one year of RN experience with clinical expertise to deliver quality patient care. Current driver’s license and ability to spend ~20% of the day driving to/from patient locations.
