Key Responsibilities\n

• Conduct comprehensive evaluations and assessments of children’s needs in collaboration with families and a multidisciplinary team.

• Participate in the development and implementation of Individualized Family Service Plans (IFSP), ensuring that each plan is tailored to meet the unique needs of the child and family.
\n
• Regularly monitor and assess the effectiveness of interventions and services provided, adjusting them as necessary based on progress toward developmental outcomes.

• Provide training and ongoing support to families, equipping them with tools and strategies to promote their child's development in a natural learning environment.

• Implement evidence-based practices and interventions while adhering to all applicable Federal and State regulations.
\n
• Engage in continuous communication with team members to promote healthy collaborative relationships and a positive environment for children.

• Utilize the coaching model to empower families in making informed decisions regarding their child's developmental needs.

• Document all services provided and maintain thorough records to ensure compliance and proper tracking of services delivered.

Patient Population / Client Focus\n

Your role as an LCSW will primarily focus on infants and toddlers experiencing developmental delays or disabilities. You will work closely with families to address a range of needs encompassing physical, cognitive, social/emotional, and communication skills. By providing high-quality support and advocacy, you will help enhance the developmental outcomes for children during their critical early years.

Qualifications\n

• Master's Degree or Doctorate in Social Work from an accredited institution.

• Current licensure as a Clinical Social Worker (LCSW) by the Department of Public Health.

• A valid driver's license to facilitate occasional travel for in-home visits or training needs.

• At least one year of professional experience working with children in an early intervention setting.

• Strong understanding of and commitment to utilizing Natural Learning Environment Practices.
