Key Responsibilities\n

Patient Population / Client Focus\n

You will primarily work with patients who have both mental health diagnoses and chronic medical conditions. This includes individuals navigating the challenges of anxiety, depression, and trauma, as well as conditions like diabetes or heart disease. Your role will focus on understanding how these conditions interact, allowing you to develop tailored interventions. This diverse patient base requires an understanding of various cultural and socio-economic backgrounds, necessitating sensitivity to each individual's unique experiences.

Qualifications\n

• A Master’s or Doctoral degree in Social Work, Marriage and Family Therapy, Mental Health Counseling, or Psychology from an accredited institution.

• Valid state licensure as a Licensed Mental Health Counselor (LMHC), Licensed Independent Clinical Social Worker (LICSW), or equivalent.

• Proven experience in delivering mental health services to individuals with co-occurring medical conditions.
