Key Responsibilities\n

• Conduct comprehensive assessments to determine patient needs and develop individualized treatment plans.

• Maintain open communication with supervisors, direct care staff, family members, and guardians concerning patient progress and any necessary adjustments to the treatment plan.

• Provide a consistently updated schedule of appointments, home visits, and observations to ensure effective monitoring of patient development.
\n
• Coordinate and conduct home visits, including in-home observations and chart reviews, as part of the treatment process.
\n
• Ensure that the implementation of treatment plans aligns with the established goals and is executed effectively.
\n
• Participate actively in Interdisciplinary Team Meetings, contributing insights and expertise to discussions regarding patient care.
\n
• Document monthly and quarterly progress notes that reflect the ongoing evolution of patient treatment and outcomes.
\n
• Facilitate training sessions for staff members to ensure competence in implementing treatment plans and maintaining proper documentation.
\n
• Provide follow-up support through interviews, home visits, and observations to ensure continued care for each individual.
\n
• Maintain comprehensive records and documentation to guarantee information is readily available and effectively shared among team members and external stakeholders.

• Act as a liaison with the wider mental health community, collaborating on initiatives that support the organization's developmental goals.

• Assist in creating and refining training materials designed to enhance program effectiveness for patients.

The LPC will work with a diverse patient population, including both adults and youth, addressing a range of mental health issues. Qualifications\n

Preferred Experience\n
