Licensed Physical Therapist - Rehabilitation Specialist

Position Overview

Functional Physical Therapy is seeking a dedicated and compassionate Physical Therapist to enhance our team in Campbell, California. This full-time role is centered around facilitating recovery and restoring movement and function for patients through tailored rehabilitation plans. You will be instrumental in assessing patients' needs, delivering hands-on treatment, and positioning them for optimal functionality in their daily lives.

Work Environment

The work setting is primarily onsite in patients' homes, which allows for personalized interaction in a familiar environment. This unique approach not only enhances the therapeutic experience but also fosters better communication and trust between the therapist and the patient. You will work Monday through Friday, from 9:00 AM to 5:00 PM, collaborating within a team that emphasizes continuous improvement and professional development.

Key Responsibilities

• Conduct thorough assessments of new patients to develop individualized rehabilitation plans that prioritize their specific mobility needs and recovery goals.
• Utilize evidence-based treatment techniques and modalities to alleviate pain and improve patients' overall function, such as therapeutic exercises, manual therapy, and functional training.
• Provide care for 1–2 patients per hour in a focused and individualized setting, ensuring each session is tailored to their progress and needs.
• Continuously monitor patient progress, reevaluate treatment strategies, and adjust therapy protocols accordingly to ensure optimal outcomes.
• Maintain detailed and accurate patient records using WebPT, and effectively communicate patient progress and treatment plans with the healthcare team to ensure cohesive care.

Patient Population / Client Focus

Our practice primarily serves patients recovering from orthopedic surgeries, sports-related injuries, and chronic pain conditions. By providing care in a home setting, we accommodate the unique needs of each individual, creating a supportive atmosphere that encourages active participation in their rehabilitation journey.

Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited institution is required. Additionally, candidates with a Master's or Doctorate in Physical Therapy will be favored.
• An active California Physical Therapy license or the ability to obtain one is necessary to practice within the state.
• Demonstrated expertise in clinical reasoning with a hands-on, patient-focused rehabilitation approach.
• Exceptional interpersonal skills to build rapport with patients, motivate them through their recovery, and engage them effectively in their treatment plans.

Preferred Experience

While a minimum of 2 years of experience in a physical therapy role is essential, candidates with experience in home health or outpatient orthopedic settings will be strongly preferred. Familiarity with documentation systems such as WebPT will contribute to your efficiency in this role.

Compensation and Benefits
