Licensed Physical Therapist (Full-Time) - Home Care Setting

Redbud Physical Therapy, Inc. is seeking a dedicated and licensed Physical Therapist to join our team in a full-time capacity. As a key member of our home care division, you will be responsible for evaluating and treating patients in their homes, working collaboratively with a multidisciplinary team to deliver personalized care plans aimed at improving functional mobility and quality of life.

Work Environment

This position is based in San Jose, California, and involves delivering therapy services directly in the homes of patients. Our full-time therapists work an average of 40 hours per week, with variable weekend schedules as mandated by patient needs. By providing care in a home setting, you will have the unique opportunity to tailor your treatment strategies to fit the individual needs of each patient within their own environment.

Key Responsibilities

• Conduct thorough evaluations of patients to develop and implement individualized treatment plans that address functional limitations and disabilities.
• Engage in effective communication with patients, families, and healthcare providers to ensure a comprehensive approach to patient care.
• Utilize evidence-based practices to enhance patient treatment outcomes, including therapeutic exercises and modalities relevant to each patient's condition.
• Document patient progress meticulously, ensuring compliance with regulatory and professional standards while maintaining accurate billing documentation.
• Provide education and training to patients and their families about treatment protocols and strategies for ongoing management of health conditions.
• Participate actively in community outreach and program development initiatives to enhance service delivery and patient engagement.
• Collaborate with interdisciplinary teams to advocate for patients' needs and facilitate a seamless continuum of care.

Patient Population / Client Focus

You will primarily work with a diverse patient population that may include individuals recovering from surgery, older adults with chronic conditions, and patients requiring rehabilitation after injury. The ability to interact respectfully and effectively with people from diverse backgrounds and cultures will be essential in ensuring personalized care and achieving optimal outcomes.

Qualifications

• A Bachelor's Degree in Physical Therapy is required.
• A valid State License in Physical Therapy.
• Basic Life Support (BLS) certification.
• Strong verbal and written communication skills.
• Proficiency in computer skills and familiarity with electronic health record (EHR) systems.

Preferred Experience

• Prior experience in home health care or outpatient orthopedic settings is preferred.
• Experience in using electronic documentation systems.
• Skills in coaching and mentoring support staff and students.

Compensation and Benefits
