In this full-time, onsite position, you will be instrumental in providing holistic health care to individuals and families within our community. Work Environment

The position is based within a hospital setting, where you will collaborate with fellow healthcare professionals in a fast-paced yet supportive environment. Your work schedule will primarily be Monday through Friday, from 9:00 a.m. to 6:00 p.m., with the possibility of evening clinics extending to 8:00 p.m. Occasionally, you may participate in community outreach events or health fairs on weekends, totaling an average of 40 hours per week.

Key Responsibilities

• Conduct thorough health assessments, including physical exams and medical history evaluations, for individuals and families.
• Diagnose and manage both acute and chronic health conditions, ensuring a focus on preventive care and health promotion strategies.
• Develop and implement tailored health education programs in collaboration with community organizations to enhance awareness and promote healthy lifestyles.
• Provide culturally competent care, actively advocating for patients’ rights and access to necessary healthcare services.
• Coordinate referrals and manage follow-up to ensure patients receive comprehensive care tailored to their ongoing health needs.
• Maintain accurate and confidential medical records, ensuring compliance with all regulatory standards and protocols.
• Engage actively in community outreach initiatives, representing Cherish Center Wellness in health fairs and public health awareness campaigns.
• Remain informed about emerging community health challenges, evidence-based practices, and welfare policies affecting health equity.

Patient Population / Client Focus

In this role, you will serve a diverse patient population in Galveston, characterized by a mix of socioeconomic backgrounds and health disparities. You will be tasked with identifying barriers to care for vulnerable groups and leveraging local resources to address these gaps. Your focus will extend to individuals and families across all age ranges, promoting a family-oriented approach to health that fosters trust and rapport.

Qualifications

• Current certification as a Nurse Practitioner recognized by relevant accrediting bodies.
• An active state nursing license must be maintained throughout your employment.
• A Master’s degree in Nursing from an accredited program is required for this position.
• A minimum of four years of clinical experience is necessary, preferably in community health or public health settings.
• Exceptional interpersonal skills, allowing for effective communication and the ability to work efficiently with a diverse client population.
• A strong commitment to social justice, health equity, and advocating for underserved communities.

Preferred Experience

Preference will be given to candidates who have previous experience working within hospital systems or community clinics, particularly those who have participated in public health campaigns. Candidates with proficiency in electronic health record (EHR) systems will also find this beneficial.

Compensation and Benefits

This position offers a competitive salary ranging from $120,000 to $220,000 annually, commensurate with experience and qualifications.
