Work Environment\n

This position is primarily remote but is situated within a detox center setting that allows for occasional on-site collaboration with healthcare professionals. The work environment promotes flexibility and encourages innovative approaches to therapy, assessment, and client engagement. You will work Monday through Friday, from 8:00 AM to 4:30 PM, with the possibility of occasional evening or weekend hours based on client needs. On-call availability may also be required.

Key Responsibilities\n

• Conduct comprehensive assessments of seniors’ psychosocial needs by evaluating emotional, mental, and social factors influencing their well-being.

• Collaborate closely with clients and their families to develop personalized care plans that address specific goals, preferences, and challenges they may face.

• Provide empathetic counseling and emotional support, guiding clients through life transitions, mental health challenges, and other significant life events.
\n
• Actively advocate for clients’ rights and their access to various community resources, ensuring they receive the most appropriate support and services.
\n
• Work collaboratively with interdisciplinary teams, including medical staff, caregivers, and other professionals, to ensure a holistic approach to client care.

• Conduct regular client visits to monitor well-being, assess living conditions, and address any safety concerns or urgent issues.
\n
• Facilitate effective communication and mediate conflicts among family members involved in the client’s care, ensuring everyone’s concerns are heard and addressed.
\n
• Educate clients and families about available community resources, support groups, and programs to enhance social integration and psychological well-being.
\n
• Maintain meticulous and accurate documentation regarding client assessments, care plans, progress notes, and all relevant interactions in compliance with ethical standards.

• Stay updated on current best practices in social work and mental health policies to ensure a high standard of care.

Qualifications\n
