Pediatric Care Navigator - Social Worker
eQHealth Solutions
eQHealth Solutions is looking for a professional to perform telephonic and onsite assessments for medically complex pediatric recipients. You will contribute to developing comprehensive care plans and work closely with multidisciplinary teams to ensure quality patient care. The role includes evaluating care plans, communicating changes to families and healthcare teams, and coordinating necessary home care needs.
