Key Responsibilities\n

• Conduct thorough evaluations and assessments to determine the needs of children and families facing end-of-life challenges.

• Participate in the development and implementation of Individualized Family Service Plans (IFSP), ensuring each plan is tailored to the unique needs of the family.

• Monitor and assess outcomes as part of a multidisciplinary team, facilitating communication and collaboration among healthcare providers.
\n
• Provide emotional support and essential resources to families, empowering them during their caregiving journey.
\n
• Adhere to all federal and state regulations, ensuring compliance in all aspects of care delivery.
\n
• Utilize evidence-based practices and the Natural Learning Environment model, guiding interventions that are contextualized within families' day-to-day lives.
\n
• Develop strong rapport with children, families, and team members to foster a supportive and healthy learning environment.
\n
• Facilitate the implementation of strategies and recommendations to enhance child development within family contexts.
\n
• Provide families with critical information, skills training, and support that addresses their specific concerns and enhances their child's growth.

• Prioritize the safety and well-being of all staff and individuals through compliance with established health standards.

Patient Population / Client Focus\n

This role primarily focuses on assisting families with young children who are experiencing disabilities, delays, or are facing terminal illnesses. You will collaborate with parents and guardians to ensure supportive, integrative care that promotes both child development and emotional wellness in the context of hospice care. Working with infants and toddlers, you will provide interventions that yield long-term benefits across various developmental domains including physical, cognitive, social, emotional, and communication skills.

Qualifications\n

• A Master's Degree or Doctorate in Social Work is required.

• Current licensure as a Licensed Clinical Social Worker (LCSW) in accordance with Department of Public Health regulations.
