This is a full-time remote position based out of our Hospice Center in Lancaster, California. Key Responsibilities\n

• Conduct virtual comprehensive psychosocial assessments to evaluate each client's strengths, needs, and challenges associated with their mental health and hospice circumstances.

• Provide evidence-based therapy and counseling services for individuals, families, and support groups, drawing on therapeutic modalities that resonate with the client’s personal experiences.

• Collaborate closely with healthcare providers, including physicians and nurses, to coordinate integrated care, ensuring a holistic approach to the client's well-being and treatment plan.

• Advocate for clients' rights and facilitate their access to vital community resources and support services, improving their capacity to manage psychological distress related to serious illness.

• Document client interactions, treatment progress, and outcomes meticulously in electronic health records, ensuring that all information is current and compliant with HIPAA regulations.

Patient Population / Client Focus\n

The LCSW will primarily work with patients and families within the hospice setting, addressing complex emotional challenges and facilitating coping mechanisms as clients navigate end-of-life issues. This requires sensitivity to their emotional needs, ability to provide grief support, and an understanding of the unique dynamics of hospice care.

Qualifications\n

• A Master's degree in Social Work from an accredited program is required to meet licensure standards.

• Current licensure as a Licensed Clinical Social Worker (LCSW) in California.

• A minimum of two (2) years of post-licensure experience providing clinical social work services, preferably in chronic illness, hospice, or palliative care settings.
