Work Environment

Key Responsibilities

• Conduct comprehensive evaluations of patients, integrating thorough history reviews and specific assessments to create personalized treatment plans.
• Clearly communicate evaluation findings and treatment strategies to patients and their families, fostering collaborative goal-setting.
• Utilize evidence-based practices to develop and implement treatment protocols that respond to each patient's unique presentation and evolving healthcare needs.
• Maintain open lines of communication with fellow healthcare professionals, recognizing and documenting patient condition changes that may necessitate adjustments to treatment plans.
• Oversee appropriate discharge planning and follow-up care while meticulously documenting all patient interactions and care activities, ensuring all records are legible and complete.
• Demonstrate effective time management skills to meet or exceed productivity expectations while maintaining punctuality and accuracy in scheduling.

Patient Population / Client Focus

Qualifications
