Work Environment

This position is primarily onsite, allowing you to deliver therapeutic services in the home environment of our patients. You will work closely with an interdisciplinary team, including nursing staff and aides, to implement and refine patient treatment plans. As a part of a cohesive team dedicated to home care, you will be equipped with the tools necessary to make a direct impact on patient rehabilitation and overall wellbeing.

Key Responsibilities

• Direct and oversee physical therapy treatment aimed at preventing disability and relieving pain while promoting improved muscle function and mobility in alignment with the established care plan.
• Conduct comprehensive evaluations of patients, utilizing diagnostic tests to assess muscle, nerve, joint, and functional abilities, which inform individualized treatment strategies.
• Implement treatment protocols, including manual and non-manual exercises, traction, and massage therapies, tailored to meet each patient's needs.
• Regularly evaluate home environments, recommending adaptations that enhance patient independence and safety.
• Work collaboratively with physical therapy assistants, ensuring effective implementation of therapy standards and compliance with the treatment strategies.
• Document all clinical evaluations, care plans, and progress notes meticulously, making them available for interdisciplinary review and compliance.
• Participate in case conferences, contributing insights and collaborating with healthcare team members to provide holistic patient care.
• Consult with patients and their families about ongoing therapy, fostering understanding and compliance with their rehabilitation program.
• Stay engaged with community resources that can benefit patient recovery and monitor changes in patient conditions to adapt care plans accordingly.
• Complete all administrative requirements, including timely documentation and submission of clinical records in accordance with company policies.

Patient Population / Client Focus

The primary focus of this role is on adult and elderly patients receiving home-based therapy. Many of these individuals deal with chronic conditions, post-operative rehabilitation, or require assistance in regaining mobility and independence following injury or illness. Your expertise will be vital in facilitating their recovery process, relying on empathy and effective communication to motivate them throughout their treatment journey.

Qualifications

• Must possess a Bachelor's Degree in Physical Therapy from an accredited program.
• A valid state PT license in good standing is required.
• Current CPR certification and a valid driver's license with automobile insurance are critical for this role.
• Demonstrated ability to manage complex patient cases with creativity, critical thinking, and the ability to adjust to diverse challenges.
• A commitment to clinical excellence and integrity in delivering care is essential.

Preferred Experience

• A minimum of one (1) year of experience as a physical therapist in an acute care or home care setting is preferred.
• Experience supervising Physical Therapy Assistants is an advantage, as well as familiarity with community-based resources and client advocacy.
• Innovative approaches toward patient treatment and a track record of successful patient outcomes are highly regarded.
