Work Environment

This role will be performed onsite at the patient's home, providing a unique opportunity to deliver care in a familiar and comfortable environment. Key Responsibilities

• Direct physical therapy treatment to prevent disability, relieve pain, and restore functional mobility in compliance with treatment plans.
• Supervise and guide Physical Therapy Assistants to guarantee the adherence to care plans and enhance patient support.
• Conduct comprehensive evaluations using diagnostic and prognostic tests to assess patients' functional abilities.
• Assure that the home environment is conducive to rehabilitation; make tailored recommendations to enhance patient independence.
• Develop and administer both manual and therapeutic exercises, while inspiring patients through encouragement and support in non-manual activities.
• Utilize physical agents as part of treatment, including traction and massage, to aid recovery.
• Evaluate, fit, and adjust prosthetic and orthotic devices, proposing modifications when necessary in collaboration with physicians.
• Participate in the interdisciplinary team by contributing to care plan discussions, evaluations, and adjustments as needed.
• Monitor and document patients' progress meticulously, noting any changes in their condition to ensure optimal care.
• Educate patients and their families on the physical therapy process, empowering them in their rehabilitation journey.
• Complete necessary clinical documentation accurately and promptly, including initial evaluations, progress notes, and discharge plans.
• Provide on-site functional assessments and community resource evaluations as part of ongoing patient care management.
• Take initiative to complete additional tasks that support organizational objectives, ensuring high-quality patient outcomes.

Patient Population / Client Focus

The Physical Therapist will primarily work with a diverse range of home care patients, including those recovering from surgeries, injuries, or chronic illnesses. Our patient demographics include elderly clients, individuals with mobility challenges, and those seeking assistance after hospitalization. A strong emphasis is placed on personalized care, addressing the unique needs and abilities of each patient to ensure effective rehabilitation.

Qualifications

• A Bachelor's degree in Physical Therapy from an accredited program.
• Active and unencumbered physical therapy licensure in Virginia.
• Current CPR certification to ensure patient safety in emergency situations.
• A valid driver's license and proof of automobile insurance to facilitate travel to patient homes.
• Exceptional communication and interpersonal skills, with a strong passion for patient care and clinical excellence.
• A track record of embracing innovative thinking and taking ownership of decisions made in clinical settings.

Preferred Experience

• At least one year of experience as a physical therapist in either an acute care or home care setting is preferred but not required.

• Familiarity with documentation requirements for home healthcare services, including OASIS assessments.

Compensation and Benefits

The annual salary for this position ranges from USD 100,000 to 120,000, commensurate with experience and qualifications.
