Key Responsibilities

• Conduct detailed patient evaluations that include examining medical histories, performing specific assessments, and creating customized treatment plans based on evaluation outcomes.
• Communicate evaluation results and treatment strategies to patients and their families, actively involving them in goal-setting processes.
• Execute treatment protocols with proficiency, adapting methods per patient condition while remaining aligned with evolving healthcare practices.
• Collaborate with other healthcare professionals to ensure comprehensive care, documenting changes in patient status that may necessitate adjustments in treatment plans.
• Develop discharge plans and oversee follow-up care, ensuring clinical documentation is accurate and timely to reflect a clear patient care trajectory, including evaluations, progress notes, and discharge summaries.
• Manage time effectively, adhering to productivity benchmarks, and maintain an organized schedule while fulfilling participation in hospital and community outreach initiatives.
• Engage in departmental goal-setting and compliance with governmental regulations to improve service delivery.
• Initiate and pursue continuing education opportunities for professional growth and serve as a clinical instructor for current students in the program.
• Promote an outstanding patient experience by being adaptable and approachable while facilitating access to necessary services.
