Work Environment

This full-time position is offered in a home care setting, providing a mix of independence and direct interaction with patients across Fredericksburg and surrounding areas. The schedule is designed to foster work-life balance, with work hours structured from Monday to Friday. In this role, you will be expected to travel to various locations, making logistical considerations key to maintaining efficient and flexible patient care delivery. In addition to direct patient interaction, teamwork and collaboration with other healthcare professionals are integral to the home care environment.

Key Responsibilities

• Conduct comprehensive patient evaluations, reviewing medical histories and performing specialized assessments to establish treatment goals and strategies.
• Communicate evaluation results and treatment plans clearly to patients and their families, ensuring that they are actively involved in the goal-setting process.
• Implement individualized treatment protocols that reflect the findings from assessments and adapt to patients' progress and changing health needs.
• Collaborate with physicians and other healthcare providers to modify care plans as necessary based on patient condition changes.
• Oversee discharge planning, completely documenting patient progress and ensuring all care activities are timely and accurately recorded.
• Manage your schedule effectively to meet productivity benchmarks while maintaining a positive patient care experience.
• Contribute to departmental goals and objectives through active participation in quality improvement initiatives, including community outreach and education.
• Pursue continuing education opportunities actively and serve as a guiding clinical instructor for students in the field.
• Enhance patient experience through exceptional communication and a willingness to adapt to diverse patient needs.

Patient Population / Client Focus

As a Physical Therapist at Reliant Physical Therapy Clinic, you will primarily serve patients with diverse physical therapy needs across Fredericksburg. Your expertise will cater to post-surgical patients, those recovering from injuries, and individuals managing chronic conditions. The focus will be on personalized treatment plans that address each patient's specific challenges and capabilities, ensuring they receive holistic and comprehensive care.

Qualifications

• Doctorate degree in Physical Therapy (DPT) required.
• A valid DPT license in Virginia and any additional state-specific licenses as needed.
• Current certification in BLS for Healthcare Providers from the American Heart Association required.
• Ideal candidates should demonstrate a commitment to ethical practice and continual learning.

Preferred Experience

• A minimum of 2 years of relevant experience in physical therapy is preferred but not mandatory.
