Key Responsibilities

• Conduct comprehensive evaluations of patients recovering from injuries, surgeries, or chronic conditions within their own homes.
• Develop and implement individualized treatment plans utilizing techniques such as therapeutic exercises, manual therapy, and modalities to alleviate pain and improve functional mobility.
• Document patient assessments, treatment sessions, and progress toward therapy goals accurately in medical records.
• Modify treatment plans as necessary based on continuous assessments and changing patient needs.
• Educate patients, families, and caregivers on treatment protocols, assistive devices, and self-management strategies to promote independence.
• Engage in professional development by attending workshops, and training sessions, and staying current with research and best practices in physical therapy.

Patient Population / Client Focus

Your role will involve working with diverse patient populations, including children, adults, and the elderly, many of whom have unique conditions requiring specialized care. Typical diagnoses range from post-operative rehabilitation to neurological conditions. Your contributions will have a significant impact on their rehabilitation journeys, enhancing their ability to regain mobility and improve their overall health.

Qualifications

• A Doctorate degree in Physical Therapy (DPT) is required.
• Current state licensure in Physical Therapy is essential.
• Exceptional verbal and written communication skills are a must for effective patient and interdisciplinary communication.
• Strong computer skills, including proficiency in electronic medical records systems.
• Ability to work independently and demonstrate flexibility in varying home environments.

Preferred Experience

Compensation and Benefits
