Work Environment

This role will be based in a home care setting within Fredericksburg, allowing Physical Therapists to engage directly with patients in their own environments. Key Responsibilities

• Conduct comprehensive evaluations that include thorough reviews of medical history, assessments, and detailed documentation of findings to formulate effective treatment plans tailored to individual patient needs.
• Communicate evaluation results and treatment strategies clearly with patients and their families while actively involving them in the goal-setting process to promote engagement and adherence.
• Implement treatment protocols with precision based on evaluation findings, adapting approaches to meet the unique presentations of patients and to align with modern healthcare standards.
• Collaborate with a multidisciplinary team of healthcare professionals, recognizing and documenting any changes in patient conditions that necessitate adjustments in care plans or further medical consultations.
• Oversee discharge planning, ensuring that follow-up care is adequately arranged, and maintain accurate documentation including focused initial evaluations, progress notes, and discharge summaries to trace the patient's recovery timeline.
• Demonstrate exceptional time management skills by adhering to a structured schedule while fulfilling productivity expectations, contributing to administrative duties that include participation in community outreach and marketing initiatives.

Patient Population / Client Focus

The primary patient population will consist of individuals requiring rehabilitation services in their homes, encompassing a range of conditions including post-surgical recovery, chronic pain management, and mobility impairments. As a Physical Therapist at Pathways, you will play a crucial role in empowering these individuals to regain functional independence, enhance mobility, and improve overall wellness through targeted therapeutic interventions.

Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited program.
• Current licensure as required by the state of Virginia for practicing physical therapy.
• American Heart Association's Basic Life Support (BLS) certification for Healthcare Providers.
• Certification in specialized therapeutic techniques may be beneficial but is not required.

Preferred Experience

• A minimum of 2 years of relevant clinical experience is preferred.
• Experience working in a home care or community-based setting will be advantageous.

Compensation and Benefits
