Work Environment

This full-time position requires a hands-on approach in a home care setting, where you will travel to various patient residences in Fredericksburg and surrounding areas. The role necessitates flexibility as you will manage your schedule, typically working Monday to Friday, with rotating weekend shifts as required. Our home care environment emphasizes comfort and trust, allowing therapists to build strong relationships with patients in their familiar surroundings.

Key Responsibilities

• Conduct thorough evaluations of patients' physical abilities, setting personalized goals and devising comprehensive treatment programs based on physician referrals.
• Implement therapy interventions that are suitable for the patient's age, medical history, and functional limitations. This may involve exercises to improve mobility, strength training, pain management techniques, and education on injury prevention.
• Maintain accurate and detailed records of patient progress, including evaluations, treatment plans, and discharge summaries, utilizing industry-standard documentation software.
• Supervise and mentor Physical Therapy Assistants (PTAs) and support personnel, ensuring high-quality care delivery and compliance with therapy standards.
• Collaborate with multidisciplinary teams, including physicians, nurses, occupational therapists, and speech-language pathologists, to enhance patient outcomes and integrate therapeutic strategies into broader health care plans.
• Provide constructive feedback and insight to the leadership team to promote operational efficiency and enhance the professional development of the therapy team.

Patient Population / Client Focus

This role primarily serves clients who require home-based physical rehabilitation following surgical procedures, injury recovery, or chronic illness management. Your expertise will help optimize their functional capabilities and boost their overall quality of life through tailored intervention and ongoing assessments.

Qualifications

• A Bachelor of Science degree in Physical Therapy from an accredited institution is required.
• A valid Physical Therapy license in the State of Virginia is mandatory.
• Exceptional communication skills, both verbal and written, to interact effectively with patients, families, and interdisciplinary teams.
• Proficiency in documentation and treatment planning through electronic health record systems.

Preferred Experience

• At least two years of experience in a physical therapy role, ideally within a home care setting or similar environments.
